Periodontal (gum) disease is one of the most common—and quietest—threats to your smile. Because early gum infection rarely causes sharp toothache pain, many adults don’t realize anything is wrong until they notice pink in the sink when brushing, tender or receding gums, or persistent bad breath. Underneath the surface, however, hardened bacteria can quietly detach the gums from your teeth and dissolve the supporting jawbone. Because bone loss cannot be fully reversed once it has progressed, catching periodontal disease early and stopping it in its tracks is crucial.

At Mountainside Family Dentistry in Southeast Portland (3740 SE 122nd Ave), Dr. Anthony El Youssef, Dr. Raphael Joseph, Dr. Aziz Azar, and our gentle dental hygiene team catch gum disease early and treat it comfortably with non-surgical scaling and root planing (“deep cleanings”), cooling ultrasonic instrumentation, and soothing antibacterial pocket irrigation—halting bone loss before it can loosen your teeth or require a tooth extraction.

Why Early Periodontal Therapy Protects Your Teeth & Overall Health

✓ Stops Permanent Jawbone Loss Early: Because bone dissolved by periodontal disease does not grow back on its own, removing subgingival tartar early is the key to preserving your natural bone height.
✓ Thoroughly Numb, Gentle Care: We numb the treatment area with topical gel and gentle local anesthetic (plus Adult Oral Conscious Sedation when desired) so your visit is calm and pain-free.
✓ Shrinks Deep Gum Pockets Naturally: Removing rough subgingival tartar allows swollen red gums to heal into firm, coral-pink tissue that seals tightly back around the clean root.
✓ Conservative, Honest Follow-Up Care: We evaluate your healing on a personalized, case-by-case basis—keeping you on a standard 6-month cleaning schedule whenever possible, and only recommending 3–4 month maintenance when truly needed.

How Gum Disease Progresses: From Reversible Gingivitis to Bone Loss

Healthy gums fit like a snug collar around each tooth, with a shallow 1 to 3 mm space (sulcus) that your toothbrush and floss can easily keep clean. When plaque bacteria are left undisturbed along and beneath the gumline, periodontal disease progresses through distinct stages—and because lost jawbone cannot naturally regenerate on its own, stopping this progression early is critical:

1. Gingivitis (3–4 mm • Reversible)

Soft plaque biofilm builds up along the gumline, irritating the tissue and causing redness, puffiness, and bleeding when you brush or floss. At this stage, the underlying jawbone is still 100% intact, and professional cleaning plus home care can completely reverse the inflammation.

2. Early-to-Moderate Periodontitis (4–6 mm)

Minerals in your saliva harden plaque into dark, rough tartar (calculus) locked onto the root below the gumline. The inflamed gums detach from the tooth to form a deep periodontal pocket where toothbrush bristles cannot reach, and the body’s immune response begins permanently resorbing the supporting bone.

3. Severe Periodontitis (7+ mm • Advanced Bone Loss)

If left untreated, tartar spreads deeper down the roots and into the furcation (the space between molar roots), destroying 50% to 80% of the supporting jawbone. Teeth can shift, loosen, or require extraction and bone grafting.

How a Periodontal Deep Cleaning (Scaling & Root Planing) Works in 4 Steps

As demonstrated in our 3D clinical animation above, scaling and root planing is the gold-standard non-surgical treatment to stop early-to-moderate gum disease before severe bone loss occurs:

1. Gentle Pocket Probing & Comfortable Numbing

We use a slender, millimeter-marked periodontal probe to gently measure your gum pockets and locate hidden subgingival tartar. Before cleaning begins, we numb the quadrant comfortably with topical numbing gel and gentle local anesthesia so you can relax without sensitivity.

2. Ultrasonic Scaling with Cooling Water

A precision ultrasonic scaler tip vibrates gently against the hardened deposits below the gumline, fracturing and lifting away heavy dark tartar while a continuous mist of cooling water flushes loose debris and bacterial biofilm out of the pocket.

3. Fine Hand Root Planing

Using slender, curved Gracey hand instruments, your clinician carefully planes away any remaining residual tartar and smooths the root cementum—removing bacterial endotoxins so new plaque cannot easily cling to the root surface.

4. Soothing Antibacterial Rinse & Gum Reattachment

We flush the clean periodontal pocket from the bottom up with a soothing antibacterial irrigation solution to eliminate residual bacteria. Over the following weeks, redness subsides and healthy pink gums tighten back around the clean root, preserving your bone.

Routine Cleaning vs. Deep Cleaning & Our Conservative Maintenance Philosophy

Patients often ask why a regular polishing isn’t performed when deep pockets have subgingival tartar, and whether they will need cleanings every 3–4 months or every 6 months afterward:

Routine Cleaning (Prophylaxis) vs. Scaling & Root Planing

A routine preventive cleaning polishes tooth enamel above the gumline and within shallow 1–3 mm healthy gum collars. When tartar is calcified deep on the root surface inside a 4–6+ mm pocket, polishing only the top of the tooth leaves the infection trapped underneath—which is why therapeutic scaling and root planing below the gumline is essential.

Personalized Follow-Up: 6-Month vs. 3–4 Month Cleanings

We believe in being conservative and honest with your ongoing care. Unlike offices that automatically place every deep-cleaning patient on a 3–4 month schedule forever, we prefer to keep patients on a regular 6-month cleaning schedule whenever possible. While 3 to 4 month periodontal maintenance visits are sometimes needed for deeper or stubborn pockets, we decide on a personalized, case-by-case basis based on how your gums heal.

🩺 Honest Clinical Care & Specialist Coordination When Needed

Our doctors and hygienists perform comprehensive periodontal charting, digital X-rays, non-surgical scaling and root planing, antibacterial pocket irrigation, and personalized maintenance cleanings right here at our Southeast Portland clinic. If an evaluation shows severe late-stage bone loss (7+ mm pockets) that requires surgical periodontal flap access, osseous surgery, or complex soft-tissue gum grafting, we will always give you an honest diagnosis and coordinate seamlessly with a trusted local periodontist.

Frequently Asked Questions About Deep Cleanings & Gum Disease

Does a periodontal deep cleaning (scaling and root planing) hurt?

No! Before beginning your deep cleaning, we numb the treatment area comfortably with topical numbing gel and gentle local anesthesia so you feel zero sharp pain. We typically treat one side of the mouth at a time so half of your mouth stays un-numbed for comfortable eating afterward. For anxious adults, we also offer Oral Conscious Sedation (a prescribed pill taken before your visit with a companion driver) as well as Nitrous Oxide (laughing gas) for children.

Can jawbone lost to gum disease grow back on its own?

No—bone that has already been dissolved by chronic periodontal inflammation does not naturally grow back on its own, which is why catching and treating gum disease early is so critical. Scaling and root planing removes the bacterial tartar causing the bone loss, shrinks deep pockets as your gums heal and tighten back around the clean root, and preserves your remaining bone height.

Why is a deep cleaning usually completed in two visits instead of one?

When all four quadrants of your mouth need scaling and root planing, we typically treat the upper and lower quadrants on one side during your first visit and the other side at a second visit for two key reasons: first, we only have to numb one side of your mouth at a time so you can speak, drink, and eat comfortably right after your appointment; and second, many dental insurance plans only allow half of the mouth (two quadrants) to be treated per visit. Our team carefully checks and verifies your specific insurance plan’s rules before doing any treatment so your benefits are maximized without surprises.

Will I need cleanings every 3 to 4 months afterward, or can I stay on a 6-month schedule?

We take a conservative, honest approach and prefer to keep patients on a regular 6-month cleaning schedule whenever possible. While 3 to 4 month periodontal maintenance cleanings are sometimes needed when deeper or stubborn pockets require closer monitoring, we always decide on a personalized, case-by-case basis depending on how your gums heal and respond to treatment.

What hours do you schedule scaling and root planing (deep cleaning) appointments?

We schedule scaling and root planing appointments on weekdays until 5:00 PM: Monday through Wednesday from 8:00 AM to 5:00 PM, and Thursday through Friday from 7:00 AM to 5:00 PM. We intentionally do not schedule deep cleanings during our late evening hours (5:00–7:00 PM) or on Saturdays because during those busy times our hygienists are seeing regular preventive cleanings and moving around between rooms—and since scaling and root planing is a longer, more focused procedure, we schedule these visits before 5:00 PM on weekdays so your hygienist can sit down, focus completely on you, and give you their undivided attention.
